We have a new employment opportunity for an MDS Coordinator Assistant / Assistant MDS Coordinator with a well-respected Long-Term Care / Skilled Nursing Facility (LTC / SNF) located in Fremont, California.
 
***WELCOME BONUS:  10% at 30 days, 30% at 90 days, and 60% at 180 days
 
We are looking for a Registered Nurse (RN) who is familiar with MDS 3.0, care planning, and completing assessments in a skilled nursing environment.  A minimum of one years’ MDS experience in a nursing home environment is required.  Current, unencumbered CA nursing license and CPR certification required.
 
The Assistant MDS Coordinator will provide guidance to facility employees pertaining to RAI processes and their correlation to resident care and reimbursement.  This person must carefully follow revisions to MDS/PPS requirements and sure compliance with current regulations.
 
Assistant MDS Coordinator responsibilities include:
  • Auditing and analyzing MDS data for accuracy.
  • Participating in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.
  • Ensuring that the care plan includes measurable objectives and timetables to meet resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in assessment, and that schedule of activities is developed as required.
  • Assisting in the completion of care plan portion of the resident's discharge plan.
  • Providing feedback to leadership on the status of clinical and financial reimbursement.
